Three things

1. Sherlock got John a gift and it was an essay about how his friends hate him
2. Sherlock regrets getting him the gift and probably had a difficult time getting John something because Sherlock values his mind and information. He probably hoped that John would too. What he didn’t take into account is that he probably insulted John and his friends and in that part, Sherlock is just now realizing it.
3. Besides the fact that Sherlock insulted his friends, John smiled
